The red area at 12:00 is also clearly visible in other surveys, doesn’t look like a star to me.
Very local high concentrated pocket of dust, background galaxy or something else?

The red area is too oblong to be a star. As you say, it could be a background galaxy shining through a gap below the upper arm of the central galaxy. In the zoomed out DECaLS image one can see a few distant galaxies with the same colour as the red spot.
